Provide a completed and signed Form 1195 Identity Declaration for each applicant (upload pages 3 and 4 only).
Please ensure that you complete this form correctly and that ALL the required fields are completed, for example:
- Q2 must be the details of the person making the declaration (not the applicant)
- The declarant must tick the relevant box where asking if they are an Australian citizen
- The group number of the profession or occupation must be completed
- Q3 must be the details of the applicant with the declarant’s signature (not the applicant’s signature)
- The number of years/months that the declarant has known you must be completed
If the 1195 form has not been completed correctly or is incomplete, your application will be delayed.
Scan your original document in colour. The scan should be legible and meet our document requirements – see Attach documents to your application. If the document is more than one page, save it all as one file.
