Please check out our comprehensive privacy policy. ----- If you do not already have a Medicare card number or an IHI (Individual Healthcare Identifier), please generate your own IHI by heading to https://www.servicesaustralia.gov.au/individuals/services/medicare/individual-healthcare-identifiers/how-get-ihi. The online application is instantaneous and much faster than the form application. You do not need to have a Medicare card or be an Australian citizen/PR to do this. However you are likely to need an active Australian mobile number. If you do not, the myGovID smartphone app found on the Apple App Store (https://apps.apple.com/au/app/mygovid/id1397699449) or Google Play Store (https://play.google.com/store/apps/details?gl=US&hl=en_AU&id=au.gov.ato.mygovid.droid) might be helpful to you. Here is the link for your convenience: https://www.mygovid.gov.au Please make sure that all your personal details, including addresses, on the myGov account and the IHI application matches your official visa/document records. If you have already created a myGov account and those details do not match the visa records, do not worry, you can make a new account. You also do not need to have an active mobile number to do this. Note that if you are having difficulties obtaining an IHI online, and are only planning to visit Australia for a few weeks, this digital certificate would be sufficient to apply for hotel quarantine exemption and visit indoor venues. Just note that you will need to carry proof of ID for places to cross-check this with your vaccine certificate. The benefit of obtaining a IHI linked with a myGov is that you will be able to download your certificate onto the check in app for each state, which can be convenient if you are staying here for a period of time. ------- If you are unable to provide these documents using this site, please send them via email [email protected]

However note that your certificate will NOT be linked to your myGov account until you obtain an IHI, this can be done once you arrive in Australia directly through myGov and Medicare (this is out of our control.)           A high resolution copy of your COVID-19 vaccination certificate with brand name clearly listed in English, with or without the batch/lot number clearly displayed on it. Your certificate MUST list both doses of the COVID-19 vaccine that you were administered and the dates that they were administered. If you have an EU COVID-19 Vaccine Certificate, you will need generate and upload certificates for each dose, or provide further evidence that you have had both doses. We will not be able to process your order if it only states you had 2/2 of doses but does not provide the details for both doses.
