- In addition to your COVID-19 booster dose, I have been administered TWO doses of the one following TGA approved or recognised COVID-19 Vaccines: Pfizer COVID-19 vaccine (Comirnaty) AstraZeneca (Vaxzevria) Moderna (Spikevax) Coronavac (Sinovac) Covishield (AstraZeneca/Serum Institute of India) BBIBP-CorV (Sinopharm Beijing/Covilo) OR I have been administered a SINGLE dose of: Janssen-Cilag (COVID-19 Vaccine Janssen) [Please note that the Sinopharm WIBP/Wuhan vaccine is NOT a TGA approved vaccine.]
- Proof of booster vaccination * I have an official record of your COVID-19 booster vaccine administered to me (It must clearly list in english: your full name, date of birth, the exact vaccine brand and date that the booster was administered and the country that it was administered in.)
